The Austrian Position
Austria does not, as of 2026, have national clinical guidelines recommending DoxyPEP as a standard intervention for gay and bisexual men. The Austrian Society for Infectious Diseases and Tropical Medicine (ÖGIT) has been watching the international evidence closely but has not moved to a formal recommendation.
This broadly mirrors the position of most Central European health authorities. The concerns are the same ones raised across the continent: antimicrobial resistance, the need for more long-term resistance surveillance data, and uncertainty about population-level gonorrhoea resistance trends.
That said, Austria's infectious disease community is engaged with the evidence, and off-label prescribing is available through the right channels.
Practical Access
Via Aids Hilfe Wien
The most accessible starting point. Aids Hilfe Wien keeps up with the current clinical landscape and can advise on whether any of their affiliated prescribers are currently comfortable discussing DoxyPEP. Raise it directly at a consultation: "Ich habe über Doxycyclin als Post-Expositions-Prophylaxe gegen Geschlechtskrankheiten gelesen — können wir das besprechen?"
Via the Infektionsambulanz at AKH Wien
The infectious disease outpatient clinic at AKH is the most clinically informed hospital-based route. If you are already attending for PrEP monitoring, raise the question at your next appointment. Infectious disease specialists here are familiar with the ANRS Ipergay and US trial literature.
Via a Wahlarzt with Infectious Disease Experience
A private doctor specialising in HIV and sexual health is likely to be the most flexible route for an off-label prescribing conversation. Aids Hilfe Wien can suggest names.
Cost
Doxycycline is a generic and cheap drug — the medication itself costs approximately €10–20 per month at typical DoxyPEP usage rates. The cost is the consultation to obtain the prescription. At Aids Hilfe Wien this would be part of a regular visit. Via a Wahlarzt, the consultation fee (€80–150) is partially reimbursed by ÖGK.
Doxycycline prescriptions for standard indications (chlamydia treatment, acne, rosacea) are covered by ÖGK when prescribed nominally. Off-label DoxyPEP prescriptions are more variable in terms of insurance coverage — ask your prescriber.
What to Bring to the Conversation
- Your STI history from the past 12 months — repeated bacterial STIs are the strongest clinical indicator for DoxyPEP
- Current PrEP status
- An awareness of the resistance discussion — Austrian clinicians appreciate that you've thought about AMR, not just about personal benefit
- Specifics: the 200mg post-sex dosing protocol, the 72-hour window, the fact that it doesn't cover viral infections (HIV, herpes)
