The Austrian PrEP Situation
Austria is not France or Germany. PrEP is not free and not automatically reimbursed by public insurance (ÖGK). It is prescription-only and you pay for it — but the price of generic tenofovir/emtricitabine (TDF/FTC) has dropped significantly and is broadly capped at around €50–70 per month for a 30-tablet supply. This is manageable, though not trivial.
The system is improving: advocacy by Aids Hilfe Wien and HOSI Wien has pushed for better access, and some prescribers now operate within frameworks that reduce the out-of-pocket burden. Check the current position at aids.at for any updates since this was written.
Getting a Prescription
Route 1: Aids Hilfe Wien (Recommended)
Aids Hilfe Wien works with affiliated doctors who are experienced in PrEP prescribing and LGBTQ+ sexual health. This is the most informed and affirming route. Contact them via aids.at to arrange a PrEP consultation — they will either prescribe directly or refer you to a trusted specialist.
Route 2: Infektionsambulanz at AKH Wien
The Infectious Disease outpatient clinic at AKH (Allgemeines Krankenhaus Wien) — 4. Medizinische Abteilung (Klinische Abteilung für Infektionen und Tropenmedizin) — prescribes PrEP. This is the public hospital route: covered by ÖGK for the consultation but you still pay for the medication at the pharmacy.
Route 3: Wahlarzt (Private Doctor)
Many gay-friendly Wahlarzt (private doctors) in Vienna prescribe PrEP. You pay upfront for the consultation (~€80–150) and reclaim ~80% from ÖGK. Aids Hilfe Wien can suggest names. Outside Vienna, finding a Wahlarzt with PrEP experience varies by city.
Route 4: GP (Kassenarzt)
Some public insurance GPs will prescribe PrEP, particularly in Vienna. Quality varies considerably — some are well-informed, others are not familiar with gay men's sexual health. Aids Hilfe Wien can advise on sympathetic GPs if needed.
Where to Fill the Prescription
Marien Apotheke (Vienna)
The community pharmacy of choice. Located at Mariahilfer Straße 105, 1060 Vienna — close to Aids Hilfe Wien. Staff are experienced, non-judgmental, and well-versed in HIV and PrEP medication. They typically stock generic TDF/FTC and know the community pricing. This is the pharmacy to use in Vienna.
Other Pharmacies
Generic PrEP is available at any pharmacy with a valid prescription. If your local pharmacy needs to order it, allow a day or two. The price varies slightly between pharmacies — generics are around €50–70 for 30 tablets.
The Monitoring Cycle
PrEP requires three-monthly monitoring:
- HIV test (essential before and during PrEP)
- Three-site STI screen (throat, rectal, urethral)
- Kidney function (creatinine) — TDF/FTC can affect kidneys in a minority of users
- Hepatitis B status (at initiation and annually)
These appointments can be done at Aids Hilfe Wien (most convenient and community-appropriate) or at the Infektionsambulanz at AKH or your prescribing doctor.
On-Demand PrEP (2-1-1)
The 2-1-1 on-demand protocol — 2 tablets 2–24 hours before sex, 1 tablet the day after, 1 tablet the day after that — reduces both cost and drug load for men with less frequent sex. It is evidence-based (ANRS Ipergay trial) and should be discussed with your prescribing doctor. Aids Hilfe Wien's affiliated prescribers are familiar with it.
