The Situation
Austrian healthcare has excellent clinical quality but an unusual structure for gay sexual health: many of the most LGBTQ+-affirming and PrEP-experienced doctors practice as Wahlarzt — private doctors who you pay upfront and partially reclaim from public insurance (ÖGK) later. The public system (Kassenarzt / hospital) works, but it can be slower and more conservative.
The saving grace is Aids Hilfe Wien — the country's leading community HIV and sexual health organisation, which anchors the entire system. It runs the best testing service, facilitates PrEP prescriptions, offers counselling, and connects you to the affirming doctors and pharmacies that make navigating Austria manageable.
PrEP is not free in Austria (unlike Germany or France) — you pay roughly €50–70/month for generic TDF/FTC at the pharmacy. For everything else: testing, PEP, and vaccines, the public hospital system provides free care.

The Golden Rules
Rule 1: Aids Hilfe Wien Is Your Anchor
For testing, vaccination, PrEP guidance, counselling, and doctor referrals — start at Aids Hilfe Wien. It is faster, more affirming, and more knowledgeable than navigating the public system cold.
Rule 2: PrEP Is Not Free — Budget €50–70/Month
Austria does not reimburse PrEP through ÖGK the way Germany or France do. Get your prescription via Aids Hilfe Wien or the AKH Infektionsambulanz, fill it at Marien Apotheke (Mariahilfer Str. 105, 1060 Vienna), and budget accordingly.
Rule 3: For PEP — AKH Wien Immediately, EHIC Ready
Vienna: go to AKH Wien Notaufnahme or Klinik Favoriten. Outside Vienna: nearest Landeskrankenhaus. Free for ÖGK holders and EU/EEA EHIC holders. Go immediately — don't wait over cost uncertainty.
Rule 4: Understand the Wahlarzt System
Many of the best doctors for gay sexual health in Austria are Wahlarzt — private doctors. You pay upfront and reclaim ~80% from ÖGK. This is worth knowing before you book: it looks expensive but is partially refunded.
