You know PrEP is for HIV. DoxyPEP is for everything else.
Specifically: Syphilis, Chlamydia, and Gonorrhea. If you are part of the majority of guys who forego condoms, this is your safety net. If you have a risky night, taking this antibiotic within 24–72 hours can stop these infections from taking hold.
Now officially endorsed: In 2023, the US CDC issued formal clinical guidance recommending DoxyPEP for guys who have sex with guys and transgender women who have had a bacterial STI in the past year. This legitimizes your conversation with a doctor. Frame it that way.
The Logistics (Read First)
You cannot buy this over the counter on a Sunday morning. To use this protocol, you must "front-load" your protection. You need the bottle in your cabinet before the weekend starts.
- The Ask: Tell your doctor, "I engage in condomless sex and want DoxyPEP for presumptive treatment of bacterial STIs—this is consistent with current CDC harm reduction guidelines."
The Protocol
- The Drug: Doxycycline (200mg). Usually 2 pills of 100mg.
- The Timing: Ideally within 24 hours after sex. (Max 72 hours—effectiveness drops the longer you wait).
- The Effectiveness:
- Syphilis & Chlamydia: Reduces risk by ~70–80%.
- Gonorrhea: ~55% (Resistance is a growing concern—see note below).
- Note: Because it isn't 100% effective for Gonorrhea—and gonorrhea resistance to tetracyclines is actively increasing—DoxyPEP does not replace your quarterly 3-site testing. It lowers your odds of infection significantly, but gonorrhea can still break through. Test anyway.
On gonorrhea resistance: Doxycycline-resistant gonorrhea strains are being tracked globally. This doesn't mean DoxyPEP is useless against gonorrhea—it still provides meaningful protection for now—but resistance monitoring is ongoing. This is another reason to never skip your quarterly tests.
⚠️ The Danger Zone (Strict Rules)
Doxycycline is a strong antibiotic. It has weird rules. If you ignore them, you will hurt yourself.
1. The "Lava Throat" (Esophagitis)
NEVER take DoxyPEP right before bed. If you lie down before the pill dissolves, it can get stuck in your esophagus and burn a hole in your lining. It is agonizing.
- Rule: Take it with a full glass of water.
- Rule: You must stay upright (standing/sitting) for 30 minutes. Do not go back to sleep.
2. The "Dairy Block"
Calcium blocks the drug from working.
- Rule: Do not drink milk, eat yogurt, or take calcium supplements/antacids (Tums) for 2 hours before or after the dose.
3. The "Vampire Effect" (Sun Sensitivity)
Doxy makes your skin hyper-sensitive to UV light.
- Rule: If you go to the beach the day after taking it, you will fry. Wear SPF 50 or stay in the shade.
The Decision Matrix (When to Use It)
Antibiotics should be used for Risk Management, not as a daily vitamin (to avoid breeding superbugs).
🟢 Lower Risk (Consider Skipping Doxy)
If your partner is a regular/trusted partner who:
- Has shown you recent, valid negative test results for bacterial STIs.
- AND confirms he hasn't had other partners since that test.
- Verdict: You likely do not need DoxyPEP.
🟡 Unverified / Casual (Take Doxy)
If the partner is:
- A casual hookup where you haven't exchanged recent test data.
- Someone whose bacterial STI status you simply don't know.
- Verdict: Take the dose. This isn't a judgment on him—it's a judgment on information. No data = take the safety net.
☢️ High Risk Event (Take Doxy)
If you were in an environment where control was lost:
- Group sex / Orgies.
- Dark rooms / Saunas.
- Chemsex or parties where you may have blacked out.
- Verdict: Take the dose.
Summary Checklist
Before you pop the pills, check the following:
- Did I verify his test results? (If No -> Take it).
- Did I eat dairy recently? (If Yes -> Wait 2 hours).
- Am I about to go to sleep? (If Yes -> WAIT 30 MINUTES).
