The Czech PrEP System

PrEP in Czechia is partially reimbursed by health insurance (zdravotní pojišťovna) — but only if prescribed through an approved HIV Centrum (HIV Centre). The reimbursement situation has evolved over recent years; as of 2026, eligible gay and bisexual men pay a copayment (doplatek) of approximately 500–1,000 CZK/month (€20–40) for generic TDF/FTC after insurance contribution.

This is significantly cheaper than paying full pharmacy price (~5,000–8,000 CZK for a full unsubsidised course), making the HIV Centrum route the clear choice.

How to Access PrEP

Step 1: Go to an HIV Centrum

PrEP must be prescribed by a specialist at an HIV Centrum. A GP cannot initiate PrEP reimbursement.

Prague:

  • HIV Centrum at Bulovka — Nemocnice Na Bulovce, Budínova 2, Prague 8. The primary HIV centre for Prague and one of the most experienced in the country.
  • HIV Centrum at ÚVN — Ústřední vojenská nemocnice (Military University Hospital), U Vojenské nemocnice 1200, Prague 6. Also a strong option.

Dům Světla (ČSAP) can advise which HIV Centrum currently has shorter waiting times and can facilitate a warm referral.

Outside Prague:

Step 2: Baseline Assessment

Before starting PrEP:

  • HIV test (essential)
  • Full STI screen (three-site)
  • Kidney function (creatinine)
  • Hepatitis B status

Step 3: Prescription and Insurance

The HIV Centrum doctor issues a prescription and manages the insurance pathway with your pojišťovna. Bring your průkaz pojištěnce (insurance card). The most common Czech health insurer is VZP (Všeobecná zdravotní pojišťovna), but all major insurers have coverage frameworks for PrEP when prescribed at an HIV Centrum.

Step 4: Pharmacy

Collect at any pharmacy — the insurance code ensures you pay only the doplatek (copayment). Generic TDF/FTC brands vary; ask the pharmacist for the cheapest available.

Monitoring: Every Three Months

  • HIV test
  • Three-site STI screen (throat, rectal, urethral)
  • Kidney function

These monitoring appointments happen at the HIV Centrum. Missing them can interrupt the reimbursement pathway.

On-Demand PrEP (2-1-1)

The 2-1-1 protocol is evidence-based and many HIV Centrum doctors in Czechia are familiar with it. Raise it at your initiation appointment if daily PrEP doesn't match your sex life pattern.
