What Is DoxyPEP

DoxyPEP (doxycycline post-exposure prophylaxis) is a single 200 mg dose of doxycycline taken within 72 hours after condomless sex. Clinical trials have shown it reduces the risk of chlamydia and syphilis by approximately 70–80%, and gonorrhoea by around 50%. It does not protect against HIV — PrEP does that.

DoxyPEP is becoming a recognised prevention tool in several European countries. In Romania, it is not yet a formal protocol, but doxycycline is a widely available antibiotic and some doctors are aware of the evidence.

Current Status in Romania

There is no national Romanian protocol for DoxyPEP as of early 2026. It is not automatically offered alongside PrEP at Matei Balș or Victor Babeș. However:

  • Doxycycline is a common, inexpensive antibiotic prescribed for many conditions
  • Some infectious disease doctors at Matei Balș and Victor Babeș are aware of the DoxyPEP trial data and may discuss it with informed patients
  • ARAS staff are up to date on the evidence and can support you in raising it with your prescriber

How to Access It

Option 1 — Raise it at your PrEP follow-up

At your three-monthly Doenças Infecțioase (infectious diseases) appointment, mention that you have read about DoxyPEP and want to discuss it. Use the term doxiciclină post-expunere or simply explain the protocol. Not all doctors will be familiar, but those working closely with gay and bisexual men at major PrEP centres increasingly are.

Option 2 — Ask ARAS for support

ARAS staff (021 252 43 70) can brief you on the evidence and help you frame the conversation with a prescriber. They may be able to identify which doctors are most receptive.

Option 3 — Private prescription

A private infectious disease doctor or GP at Regina Maria or MedLife may be willing to prescribe doxycycline on a private basis. A consultation costs roughly 150–250 RON; the medication itself is cheap.

Cost

Doxycycline is one of the least expensive antibiotics:

Generic doxiciclinã (doxycycline) is the same medication at lower cost — ask specifically for the generic.

What DoxyPEP Does and Does Not Do

DoxyPEP reduces the risk of bacterial STIs:

  • Syphilis: ~80% reduction
  • Chlamydia: ~80% reduction
  • Gonorrhoea: ~50% reduction (with some resistance concerns)

It does not:

  • Protect against HIV (use PrEP for this)
  • Replace regular STI testing — three-site swabs every 3 months remain essential
  • Work if taken more than 72 hours after sex
  • Eliminate all gonorrhoea risk (some strains show doxycycline resistance)

Antibiotic Resistance

The main clinical concern with widespread DoxyPEP use is contributing to antibiotic resistance, particularly for gonorrhoea. Regular testing remains important so that any gonorrhoea infection is detected and treated with the appropriate antibiotic. Discuss the resistance question with your prescriber.
