The Bulgarian Position
Bulgaria does not have a national clinical guideline on DoxyPEP. The Bulgarian Society of Infectious Diseases has not issued a formal recommendation, and awareness of the DoxyPEP evidence base among Bulgarian clinicians is variable.
In practice, doxycycline is a widely available generic antibiotic in Bulgaria — inexpensive and obtainable at pharmacies. The clinical conversation about off-label DoxyPEP is possible but depends on finding a doctor familiar with the international evidence.
Practical Access
Via Checkpoint Sofia / Single Step Foundation
The most reliable route. Single Step Foundation (singlestep.bg) works with infectious disease specialists who are aware of international HIV prevention developments. Ask directly: "Можете ли да ме насочите към лекар, с когото мога да обсъдя профилактиката с доксициклин срещу ППИ?" (Can you refer me to a doctor I can discuss doxycycline prophylaxis for STIs with?)
Via a Private Infectious Disease Specialist
A private infectious disease specialist (частен инфекциолог) familiar with HIV prevention is the most likely to know the trial data and be comfortable with off-label prescribing.
Cost
Doxycycline is among the cheapest antibiotics in Bulgaria — approximately 10–25 BGN per month (€5–13) at typical DoxyPEP usage rates. The consultation to obtain the prescription is the main cost (typically 40–80 BGN at a private specialist).
