The Belgian Position

Belgium does not, as of 2026, have a national clinical guideline formally recommending DoxyPEP as a routine intervention for gay and bisexual men. The Belgian HRC network and the ITM in Antwerp are clinically engaged with the evidence, and off-label prescribing is available through specialist centres.

Belgium's clinical culture around HIV and sexual health is pragmatic and evidence-oriented — the HRC system has historically adopted new prevention tools relatively quickly. The ITM in Antwerp, in particular, has international standing in HIV and tropical medicine research and is among the better-informed centres in Belgium for having this conversation.

If you are enrolled in the PrEP Convention, raise DoxyPEP at your next monitoring appointment. At CHU Saint-Pierre (Brussels) and ITM Help Center (Antwerp), clinicians are familiar with the trial literature.

Cost

Doxycycline is inexpensive — the medication costs approximately €10–20/month at typical DoxyPEP usage. As an off-label prescription, Mutuelle reimbursement depends on the prescribing indication — ask your prescriber how they will code it. The consultation itself is covered at standard HRC or specialist rates.
