PrEP in Portugal is fully funded and provided free of charge through the National Health Service (SNS) in public hospitals. The system works, but getting your foot in the door can be slow if you take the traditional route of going through a primary care doctor. The "cheat code" for accessing PrEP in Portugal is to skip the standard clinic and go directly to a community-based organization for your initial screening and referral.

Who Can Get It

PrEP is indicated for adolescents (16+) and adults at increased risk of HIV. The SNS covers it for:

  • Men who have sex with men (MSM) and trans individuals.
  • Anyone who has had condomless sex in the last 6 months with partners of unknown HIV status.
  • Individuals diagnosed with an STI or who have used PEP in the past 6 months.
  • Sex workers and individuals who inject drugs.

As long as you are registered with the SNS (having a Número de Utente), the consultations, lab tests, and medication are completely free.

How to Get It

There are two main pathways into the public system, and one is vastly superior to the other.

  1. The Traditional Route (Slow): You schedule an appointment with your Family Doctor (Médico de Família) at your local Health Center (Centro de Saúde). They assess your risk and send a referral to a hospital specialist. This can take months.
  2. The Community Route (Fast): You go to a specialized NGO/community center like CheckpointLX (Lisbon), GAT, or Abraço. These centers perform the initial triage, run your baseline rapid tests, and have direct referral pathways to hospital infectious disease departments. This cuts through the primary care bureaucracy.
  3. The Hospital Appointment: Once referred (via either route), you see a specialist at the hospital. They will do full lab panels (kidneys, liver, full STI screen) and issue the prescription.
  4. The Pharmacy: Free PrEP is dispensed at the hospital pharmacy (farmacia hospitalar), not at the standard retail pharmacies on the street.

If You Can't Wait

If you don't have a Número de Utente yet, or you are stuck on a waitlist:

  • Private Sector: You can book an appointment at a private hospital or clinic (e.g., CUF, Trofa Saúde). You will pay out of pocket for the consultation and the required lab tests. You will also have to pay for the medication at a private pharmacy (it may be partially subsidized if you have a prescription, but it will not be free like the SNS).
  • Self-Sourcing: It is strictly prohibited to import prescription medications like PrEP via mail from outside the EU/EEA. Portuguese customs will impound the package. If you are traveling into Portugal, you can legally bring a 30-day supply (sometimes up to 90 days) for personal use, provided it is in its original packaging and accompanied by a valid prescription.

Non-negotiable regardless of route: You must confirm you are HIV-negative before starting PrEP. Starting PrEP with an undetected infection risks drug resistance and makes HIV much harder to treat.

What Happens After

Once you are in the SNS system, monitoring is handled by the hospital clinic.

  • The 3-Month Check-in: You will have appointments every three months to renew your prescription. This includes blood tests for HIV, kidney function (creatinine), and syphilis.
  • Three-Site Swabs: Ensure you are being swabbed for chlamydia and gonorrhoea in the throat, rectum, and genitals. If the hospital only requests a urine sample, ask for the full three-site swabs.
  • Vaccinations: The hospital clinic is also the place to verify you are up to date on Hepatitis A, Hepatitis B, HPV, and Mpox vaccines, which are generally covered.

  • On-demand (2-1-1): Widely supported by community centers like CheckpointLX and recognized by specialists.
  • Injectable (CAB-LA / Apretude): As of 2026, Europe is slowly rolling out injectable PrEP. Ask your hospital specialist about current SNS funding and eligibility for CAB-LA.
