Testing in Spain: Clinics and NGOs

The Short Version

For routine testing, community-run LGBTQ+ NGOs are the fastest and most culturally competent option. Public specialized clinics (CESS) offer full 3-site swabs for free but require appointments that book up instantly. Private clinics offer walk-in convenience for a fee.

Demand Three-Site Testing: General practitioners may only offer a urine test. You must explicitly ask for throat and rectal swabs if you have engaged in oral or receptive anal sex. See our Spanish Vocabulary Guide for the exact phrases to use.

🏛️ Top NGO & Community Clinics (Recommended)

Community clinics are designed by and for gay/bi men. They offer rapid, anonymous testing and can fast-track you into the public hospital system if you need PrEP or HIV care.

Barcelona: BCN Checkpoint

🏥 Public Specialized Clinics (CESS)

These clinics offer comprehensive, free screening but suffer from severe appointment bottlenecks.

Madrid: Centro Sanitario Sandoval (CESS)

• Cost: Free
• Requirements: Requires Madrid Health Card (SERMAS) and ID. Not anonymous.
• Testing Scope: Full 3-site swabs and bloods.

Barcelona: CAP Drassanes

• Booking: Mandatory appointment via vhc.cat/pacientsdrassanes.
• Cost: Free
• Requirements: Requires Catalan Health Card (CatSalut) and ID. Not anonymous.
• Testing Scope: Full 3-site swabs and bloods.

Demand Three-Site Testing: If you are at a general clinic outside a major hub, the doctor may only offer a urine test. You must explicitly ask them to swab your throat and rectum if you have engaged in oral or anal sex.

🔬 Private Clinics (Walk-in, Paid)

If you cannot get a public appointment and NGOs are fully booked, private clinics offer full discretion and immediate access.

Open House (Madrid & Barcelona)

Megalab / Synlab (Nationwide)

• Locations: Multiple locations in all major Spanish cities.
• Booking: Walk-in accepted.
• Cost: Varies by test (~€30 per specific PCR, up to €120 for panels).
• Requirements: ID required.
• Testing Scope: Direct-to-lab testing. You must know exactly which tests to request.
