Chemsex exists in Estonia, though it's less openly discussed than in larger Western European cities. Estonia has one contextual advantage: the country's significant experience with injectable drug harm reduction (primarily fentanyl and amphetamines) means harm reduction infrastructure — needle exchanges, counselling services, naloxone availability — is more developed than in many similar-sized countries, and benefits the chemsex community even where services aren't explicitly labelled "chemsex."

Nakkuskliinik — Harm Reduction

The Infectious Disease Clinic at Lääne-Tallinna Keskhaigla (Paldiski mnt 62) operates harm reduction services including needle exchange. They can also connect you with social workers and counselling for drug-related issues. If you're HIV-negative and using drugs during sex, the infektsionist team here are experienced, non-judgmental, and won't penalise you for honest disclosure.

PEP is available at Paldiski mnt 62 within 72 hours if you've had a potential HIV exposure.

lgbt.ee — Can refer to counsellors experienced with chemsex and substance use in the gay and bisexual male community.

The HIV Context

Estonia already has elevated HIV rates, particularly in Ida-Virumaa. Chemsex further increases transmission risk. If you're HIV-negative and using drugs during sex, PrEP is particularly important here.

🚨 Overdose & the Law

If someone is unresponsive, not breathing normally, fitting, or has collapsed — call 112 now. Put them in the recovery position if they're breathing, tell the operator what was taken if you know, and stay with them until help arrives. Getting them medical help is the only priority.

Call 112. Estonia doesn't have a specific "Good Samaritan" law giving you automatic immunity for reporting an overdose. While possession of small amounts for personal use is generally treated as a misdemeanour rather than a criminal offence, it remains illegal and carries some risk. However, the system is increasingly health-focused (such as the SÜTIK programme for alternatives to punishment), and emergency services prioritise medical care. Never let fear of getting in trouble stop you making the call — a life comes first.
