While many people use the physical Checkpoint network or visit a specialist doctor (Schwerpunktarzt), Germany also offers highly reliable options for testing from the privacy of your own home. This includes a comprehensive postal laboratory service and over-the-counter HIV rapid tests.

📦 S.A.M Health (Mein Heimtest)

s.a.m health is the most popular at-home testing service for gay and bisexual men in Germany. It is operated by the Deutsche Aidshilfe. Unlike rapid pharmacy tests, your samples are mailed to and processed by a certified medical laboratory, making it as reliable as a clinic visit.

  • What it tests for: HIV, Syphilis, Gonorrhoea, and Chlamydia.
  • Cost: It is not free, but it is community-run. A first-time full kit usually costs around €75 (which includes the mandatory initial consultation), and follow-up kits are around €59.

How it Works

  1. Registration & Consultation: You register online. Before your first kit is sent, you are required to have a brief telephone consultation with a counselor from a participating Aidshilfe checkpoint to ensure you understand the swabbing process and window periods.
  2. The Kit: You receive a kit in discreet, plain packaging. It contains materials for a finger-prick blood draw, a urine sample, and rectal/throat swabs.
  3. The Lab: You mail the samples back to the laboratory in a prepaid return envelope.
  4. Results: You will be notified of your results via SMS or your online account within a few days. If a result is reactive for an infection, a counselor will contact you directly to provide support and arrange treatment.

🩸 The Finger-Prick Reality

For both s.a.m health kits and rapid pharmacy tests, the blood collection is where most people struggle. Squeezing thick drops of blood into a tiny tube or onto a testing card can be frustrating and messy.

How to succeed at the finger-prick test:

  1. Hydrate: Drink two massive glasses of water 30 minutes before you start. Thick blood won't flow.
  2. Heat: Do 20 jumping jacks to get your heart rate up, then wash your hands in hot water for 3 minutes. Your veins need to be warm and dilated.
  3. Gravity: Stand up to do the test. Keep your hand pointing down, below your heart level.
  4. The Pierce: Use the side of your ring finger, not the sensitive center pad. Press the lancet firmly into the skin before clicking the release button.

🩸 HIV Rapid Self-Tests (HIV-Heimtest)

If you only want to check your HIV status quickly and do not need a full STI panel, you can buy an HIV-Heimtest (HIV rapid self-test).

  • Where to get it: You can buy these over-the-counter at almost any pharmacy (Apotheke), large drugstores (like dm or Rossmann), or order them online.
  • Cost: Usually between €20 and €30.
  • How it works: It is a 15-minute finger-prick blood test you perform entirely at home.

The 12-Week Window: Pharmacy rapid tests detect antibodies. They are only considered 100% conclusive for exposures that happened 12 weeks (3 months) ago or more.

Do not use a rapid self-test to check for an exposure that happened last weekend. If you had a high-risk exposure within the last 48 hours, go to a hospital emergency room immediately and ask for PEP.
