Get those three right and the system is powerful — PrEP has been covered by GKV since 2019, vaccinations against Mpox and hepatitis are included, and every major city has an Aidshilfe branch to help you navigate.
The system has quirks — long waiting lists and a quarterly billing cycle — but the community infrastructure bridges the gaps.
⚖️ The Golden Rules
Rule 1: PrEP Requires a Schwerpunktarzt — Not Your GP
To get PrEP covered by the GKV, your prescription must come from a doctor with specific HIV/infectious disease certification. Your standard GP (Hausarzt) can only issue the referral (Überweisung) you need to see the specialist.
Rule 2: PEP Requires the Notaufnahme Within 72 Hours
If you need PEP, go to the Notaufnahme (emergency department) of the nearest Uniklinik immediately. You will receive a starter pack; book a Schwerpunktarzt appointment immediately the next working day for the full course.
Rule 3: Checkpoints Are Open to All
Checkpoint community testing centres provide HIV and STI testing regardless of your insurance status. No Gesundheitskarte or GP referral is needed.
Rule 4: Track the Quartal
Germany's healthcare billing year divides into four quarters. Referrals are tied to the quarter. The best time to call clinics for new patient appointments is the first working day of a new quarter.

If you test positive for an STI and absolutely cannot face the direct message (e.g., safety concerns, extreme anxiety), you must still ensure partners are warned.
