Chemsex exists in Bulgaria — primarily in Sofia — but is less visible and less organised than in Western European cities. The combination of social stigma, weaker community infrastructure, and limited harm reduction services means gay men in Bulgaria navigating chemsex often do so with less support than elsewhere.

Checkpoint Sofia / Single Step Foundation
The primary and essentially the only LGBTQ+-aware harm reduction resource in Bulgaria for this context. Single Step Foundation (singlestep.bg) has counsellors and health workers who will discuss drug use in sexual contexts without judgment and can signpost to addiction and mental health support.
Анонимни Алкохолици / Наркотици (AA / NA — Narcotics Anonymous)
General addiction support through the international NA network. Not LGBTQ+-specific, but available in Sofia. Check na.org for the current Bulgarian meeting schedule.
Hospital Addiction Services
Addiction treatment (лечение на зависимости) is available through the public hospital system. For acute addiction concerns, the psychiatric departments at major Sofia hospitals (УМБАЛ „Александровска", УМБАЛ „Света Екатерина") can provide assessment and referral. Not LGBTQ+-specific.
Drug Checking
No established drug checking service in Bulgaria. Energy Control (Spain, energycontrol.org) accepts postal samples from Bulgaria.
PrEP in a Chemsex Context
PrEP is particularly important in chemsex contexts in Bulgaria given the country's higher background HIV prevalence. If you're regularly having chemsex, being on PrEP is not optional.
