Who Can Access Care?

Bulgaria operates a mandatory public health insurance system managed by the National Health Insurance Fund (NHIF / НЗОК - NZOK). If you are employed in Bulgaria, contributions are automatically deducted from your salary.

Tourists and short-term visitors from the EU/EEA can use their EHIC (European Health Insurance Card) to access emergency care at state hospitals (such as for PEP).

The Key Navigation Principle: Community & Private Care

While the public system covers basic care and emergencies, navigating it for LGBTQ+ sexual health can be challenging due to documented stigma, inconsistent clinical knowledge regarding gay men's health, and bureaucratic hurdles.

Because of this, the most effective navigation principle in Bulgaria is to bypass the standard public GP route for sexual health:

  1. Community Hubs: Organizations like Checkpoint Sofia and the Single Step Foundation act as the de facto primary care entry point for gay men. They offer free testing, non-judgmental counseling, and hold the networks of affirming doctors.
  2. Private Specialists: For PrEP prescriptions and full STI testing panels, many men rely on private infectious disease specialists or private laboratories (like Ramus, Cibalab, or Synevo). You pay out-of-pocket, but you bypass the stigma and gatekeeping of the public system.

Registering as a Resident

If you move to Bulgaria long-term, you must register with the National Revenue Agency (NRA/НАП) to ensure your health insurance contributions are tracked. Once insured, you select a GP (Личен лекар), though again, you may prefer to seek sexual healthcare privately or via community clinics.
