1. The Pharmacist Prescribing Law
In California, you do not need a doctor's appointment to start PrEP. Under state law, licensed pharmacists can independently prescribe and dispense up to a 60-day starter supply of oral PrEP (Truvada or generic TDF/FTC) without a prior physician's visit. Walk into a participating pharmacy, get your HIV test on the spot, and walk out with your first supply.
The testing prerequisite still applies. The pharmacist must confirm a negative HIV test before dispensing. Many participating pharmacies can run a rapid HIV test in the clinic on the same visit. Call ahead to confirm they offer this.
2. Medi-Cal Covers Almost Everything
California's Medi-Cal program is one of the most generous Medicaid expansions in the US. If your income is at or below 138% of the federal poverty level, you likely qualify—and Medi-Cal covers PrEP, STI testing, HIV treatment, vaccines, and mental health services with little or no out-of-pocket cost. - LGBTQ+ clinic density: SF, LA, San Diego, and Sacramento all have dedicated, affirming health centers.

Los Angeles: The LA LGBT Center
The LA LGBT Center (lalgbtcenter.org) in Hollywood is the largest LGBTQ+ healthcare organization in the world. They run a full-service health clinic offering PrEP, STI testing, mental health services, and HIV care, serving patients regardless of insurance status or ability to pay.

💊 The PrEP-AP Safety Net
California's PrEP Assistance Program (PrEP-AP) is a state-funded programme that covers the cost of PrEP medication and associated clinical visits for people who are uninsured or underinsured and do not qualify for Medi-Cal.
PrEP-AP is administered through enrolled provider clinics—you cannot apply directly. Ask your FQHC or LGBTQ+ clinic whether they are a PrEP-AP enrolled site when you book your first appointment.
