Accessing healthcare in Ukraine depends on your residency status. While the state system covers citizens and registered residents, the network of community NGOs provides a crucial bypass for sexual health services.

🇺🇦 Citizens and Registered Residents

The Ukrainian healthcare system has undergone significant reforms to a "money follows the patient" model, managed by the National Health Service of Ukraine (NHSU).

  1. The Family Doctor: To access free secondary and specialized care (like a dermatologist or endocrinologist), you must first sign a declaration (декларація) with a primary care family doctor (сімейний лікар).
  2. Specialized Care: Your family doctor provides electronic referrals (e-направлення) to specialists.
  3. The Exception for Sexual Health: You do not need a referral from a family doctor to visit an infectious disease doctor, a dermatovenerologist, or an AIDS Center. You can self-refer directly to these specialists for HIV/STI testing and treatment.

Internally Displaced Persons (IDPs): Due to the ongoing war, the system has adapted to allow displaced citizens to access care and receive prescriptions (including ART and PrEP) at any public hospital or AIDS Center in their current location, regardless of where their family doctor is registered.

🌍 Tourists and Visitors

Ukraine is not part of the EU, and the European Health Insurance Card (EHIC) is not valid here.

  • Emergency Care: True emergency care (e.g., calling an ambulance for a life-threatening situation) is provided free of charge, but follow-up hospital care will be billed.
  • Sexual Health Services: As a tourist, you can access free rapid HIV/STI testing through NGOs. However, for treatment (like PrEP or STI antibiotics), you will generally need to use private clinics and pay out of pocket, as you are not registered in the state eHealth system.
  • Insurance: You must carry comprehensive travel health insurance that explicitly covers medical expenses in Ukraine.

🚪 The Key Navigation Principle

For sexual health, bypass the family doctor. Go directly to community NGOs (like Alliance Global or 100% Life) or straight to the state Trust Cabinets / AIDS Centers. The NGO route is almost always the most affirming, efficient, and discreet way to enter the system.
