The public healthcare system in Portugal is the Serviço Nacional de Saúde (SNS). The SNS provides comprehensive medical care that is "tendentially free," meaning most services are free or involve very low standard co-payments (taxas moderadoras).

How National Health Insurance Works

Unlike insurance-based systems in other countries, the Portuguese SNS is residence-based and funded through general taxation. Any legal resident of Portugal is entitled to access the SNS under the same conditions as Portuguese nationals.

Once registered, you are assigned an SNS User Number (Número de Utente), which is required to access public clinics (Centros de Saúde), hospitals, subsidized medications (including PrEP), and free HIV/STI treatment. Registration is completed at your local Centro de Saúde based on your official residential address.

How Foreigners Can Register

Registration is based on legal residency, not nationality. However, the documentation required to prove your residency differs between EU and non-EU citizens.

To register, you must go in person to your local health center (Centro de Saúde) bringing your Passport/ID, your Portuguese Tax Number (NIF), and proof of address (often a certificate from your local Junta de Freguesia). The most critical document is your proof of legal residency:

EU/EEA/Swiss Citizens

Citizens of the EU exercising their right to free movement must obtain a registration certificate:

  • CRUE (Certificado de Registo de Cidadão da União Europeia): You must obtain this from your local municipal council (Câmara Municipal) after being in Portugal for 3 months. You use this certificate to register with the SNS.
  • Temporary stays: You can use your European Health Insurance Card (EHIC) for necessary care without needing to register fully.
  • Pensioners: You can bring an S1 form from your home country to register for reciprocal healthcare coverage.

Non-EU Citizens

Non-EU citizens must demonstrate their legal right to reside in Portugal through immigration authorities:

  • Residence Permit (Autorização de Residência): You must present a valid residence permit issued by AIMA (formerly SEF).
  • Pending Applications: If your residence permit is still processing, some health centers will accept your manifestation of interest (Manifestação de Interesse) or appointment confirmation, but this varies significantly by location and staff discretion. It is highly advised to carry private health insurance until your SNS registration is finalized.

Portugal operates a centralized health hotline called SNS 24. This line provides clinical triage, counseling, and administrative information (including helping you find your local Centro de Saúde). You should call this number for non-emergency medical issues before going to a clinic.
