Ireland's testing infrastructure is highly efficient if you know which channel to use. Because physical clinic slots are highly contested, the HSE operates a free, national postal testing service to keep asymptomatic people out of the waiting rooms.

📦 The Postal Route: SH:24

If you do not have symptoms, do not try to book a physical clinic appointment. Use the free home-testing service.

The HSE contracts the UK-based provider SH:24 (sh24.ie) to run the national postal system.

  • How it works: You order a kit online. It arrives in a discreet envelope. You collect your own blood (finger-prick), urine, and swabs (throat and rectum), and mail it back in a prepaid envelope.
  • Results: Sent via text message, usually within 72 hours of the lab receiving the kit.
  • Cost: 100% Free.

The 8:00 AM Rule: Due to high demand, SH:24 sometimes hits its daily quota of kits. If the website says "We have reached our limit for today," log on exactly at 8:00 AM the next morning to secure one.

🩸 The Finger-Prick Reality

The kit will contain swabs for your throat and rectum, a urine tube, and a finger-prick blood lancet to check for HIV and Syphilis (and sometimes Hepatitis/kidney function if you're on PrEP).

The blood collection is where most people fail. Squeezing thick drops of blood into a tiny tube or onto a card can be frustrating and messy.

How to succeed at the finger-prick test:

  1. Hydrate: Drink two massive glasses of water 30 minutes before you start. Thick blood won't flow.
  2. Heat: Do 20 jumping jacks to get your heart rate up, then wash your hands in hot water for 3 minutes. Your veins need to be warm and dilated.
  3. Gravity: Stand up to do the test. Keep your hand pointing down, below your heart level.
  4. The Pierce: Use the side of your ring finger, not the sensitive center pad. Press the lancet firmly into the skin before clicking the release button.

🏥 The Clinic Route

If you have active symptoms (a rash, discharge, a sore) or need a PrEP initiation assessment, the postal kit cannot help you. You must go to a physical clinic.

GP Limitations: If you visit a standard GP for STI testing, they will charge you a consultation fee (around €60) and often only do a urine or blood test. Many GPs will not perform the rectal and throat swabs required for gay men. Always use an STI clinic or SH:24 if possible.
