Northern Ireland's health system — Health and Social Care Northern Ireland (HSCNI) — operates distinctly from NHS England, Wales, and Scotland. The free-at-point-of-use principle and the sexual health firewall apply here as they do across the UK — for those foundations, see UK: The GUM Clinic & The Firewall.
What differs is that the system requires more active navigation, particularly outside of Belfast, due to funding constraints and geography.
🏥 HSC Trusts & GUM Clinics
HSCNI is organised into Health and Social Care Trusts. Sexual health services are commissioned at the trust level, meaning the density and speed of provision varies significantly depending on where you live.
Belfast: This is the centre of sexual health provision. The primary GUM clinic is at the Royal Victoria Hospital under the Belfast HSC Trust. This is the most LGBTQ+-experienced clinic in Northern Ireland and the main hub for PrEP, HIV management, and comprehensive STI screening.
Outside Belfast: Provision thins out considerably. Check sexualhealthni.info for the current list of clinics across the Northern, South Eastern, Southern, and Western trusts.
Rural Access: If you are in a rural area, your most practical options are often travelling to Belfast GUM, using the postal home testing service for routine screening, or considering the IWPN private import route if you face significant delays for PrEP.
🏳️🌈 LGBTQ+ Navigation & Support
The Rainbow Project (rainbow-project.org) is Northern Ireland's primary LGBTQ+ health organisation. Based in Belfast, they provide sexual health outreach, mental health support, and critical navigation into the HSC system. For anyone encountering the NI system for the first time — or dealing with an HIV diagnosis — they are the best first point of contact outside the clinic.
Cara-Friend (cara-friend.org.uk) provides additional LGBTQ+ counselling, peer support, and information across Northern Ireland.
Both organisations are highly experienced with chemsex-related enquiries and can connect you to appropriate, non-judgemental support.
🌍 The Cross-Border Option
For residents near the border (e.g., South Down, Fermanagh, South Armagh), or those facing prohibitive wait times for Belfast appointments, Dublin is a practical alternative.
The GUIDE Clinic at St James's Hospital (guideclinic.ie) in Dublin is the Republic of Ireland's leading LGBTQ+-affirming sexual health service. UK residents can access it as a private patient, with costs typically remaining modest.
The GUIDE Clinic operates within the Irish healthcare system. For Irish services, including PrEP access in Dublin, see the Republic of Ireland country guide. Using Dublin for private sexual healthcare does not affect your HSC entitlements in Northern Ireland.
💬 Anonymous Partner Notification
If you test positive for an STI, you can notify your partners completely anonymously. You do not need to face a difficult conversation if you feel unsafe or anxious.
- Clinic Health Advisers: HSC clinics employ Health Advisers. You can give them your partners' phone numbers, and they will text them anonymously to say they have been exposed to an STI and should get tested. They will never reveal your identity.
- SH:24: The postal testing portal for Northern Ireland has built-in anonymous notification tools.
- Let Them Know: A free online service (letthemknow.org.uk) allows you to send anonymous texts or emails.
