Because Spain's healthcare system is decentralized, the rules around which vaccines are free for gay men change depending on which Autonomous Community you live in.

💉 The Free Vaccine Stack

If you are a man who has sex with men, you are generally entitled to the following vaccines for free via the public system (Seguridad Social), provided you have your regional health card:

Do not ask your GP for these. The easiest way to get them is to ask the doctor during a routine visit to a specialist CESS clinic (like Sandoval in Madrid or Drassanes in Barcelona) or via an NGO referral. They hold the specialized stock.

💳 The HPV Problem

The Human Papillomavirus (HPV) vaccine prevents anal and throat cancer, as well as genital warts.

Spain provides the HPV vaccine for free to school-aged children, but its policy for adult men is highly restrictive and varies by region.

In most of Spain, the HPV vaccine is NOT free for adult gay men unless you are living with HIV or under the age of 26.

If you are over 26, HIV-negative, and want the vaccine (which you should, as it prevents cancer), you will likely have to pay for it privately.

  • You must get a private prescription from a doctor.
  • You buy the vaccine (Gardasil 9) from a normal street farmacia.
  • It costs roughly €170 per dose, and you need three doses over six months.
  • You then take the physical box to a private clinic or your local health center to have a nurse inject it.
