Spain's public health system (Seguridad Social) requires you to visit a clinic for STI testing. There is no national, free postal testing system. If you want to test from the privacy of your home, you have to use the private market or over-the-counter pharmacy options.
🩸 The Pharmacy HIV Test (Autotest VIH)
If you only need to check your HIV status, Spain makes this incredibly easy. In 2017, Spain legalized the over-the-counter sale of HIV self-testing kits.
How to get it: Walk into almost any physical pharmacy (farmacia)—look for the green cross—and ask for an "Autotest VIH".
- Cost: Approximately €25 to €30.
- Prescription: None needed. You do not need a health card, and you do not need to give your name.
- How it works: It is a 15-minute finger-prick blood test.
The 3-Month Window: The Autotest VIH detects antibodies. It is only considered 100% conclusive for exposures that happened 3 months (12 weeks) ago or more. Do not use it to check an exposure that happened last weekend.
📦 Private Full-Panel Postal Kits
If you are buying PrEP online and need to monitor your kidney function, Syphilis, Chlamydia, and Gonorrhea without dealing with hospital waitlists, you can buy comprehensive postal kits from private laboratories.
You order the kit online, collect your own blood (finger-prick), urine, and swabs at home, and mail it back to the lab.
| Provider | URL | What they offer | Cost Estimate |
|---|---|---|---|
| LetsGetChecked | letsgetchecked.com/es | Ships internationally to Spain. Offers comprehensive STI panels including 3-site swabs and bloods. | €100 - €150 |
| OpenHouse | openhouse.es | Madrid-based clinic that offers discrete postal testing kits mailed anywhere in Spain. | €80 - €130 |
| Better2Know | better2know.es | Offers "Home Testing" kits mailed to Spanish addresses, though they primarily push you toward their private partner clinics. | €150+ |
The Treatment Catch: If a private postal kit flags you as positive for Chlamydia, Gonorrhea, or Syphilis, they cannot legally mail you antibiotics in Spain. You will still have to take your positive result to a public CESS clinic, NGO (like BCN Checkpoint), or a private doctor to receive the actual medication or injection.
🤝 NGO Mail-Out Campaigns
Occasionally, regional NGOs (like Apoyo Positivo or Stop Sida) will run publicly funded campaigns where they mail free HIV or Syphilis oral swab tests to individuals living in rural areas or outside major cities.
These are heavily dependent on grant funding and are not a permanent infrastructure. Check their websites directly to see if a campaign is currently active in your Autonomous Community.
