Madrid and Barcelona dominate the conversation around sexual health in Spain, but millions of guys live elsewhere. Because Spain's healthcare is devolved to its 17 Autonomous Communities, your experience in Valencia will be wildly different from your experience in Andalusia.

🤝 The Role of Local NGOs

Outside the two main capitals, the public system is often slower and less accustomed to high-volume gay sexual health needs. Therefore, local NGOs become even more critical. They provide free rapid testing and know exactly which local hospital doctors are friendly and efficient.

🏥 Navigating the Public System

If you do not have an NGO nearby, you must rely on the standard public route (Seguridad Social).

  1. The Primary Care Hurdle: You usually must start with an appointment with your family doctor (médico de cabecera) at your local health center (Centro de Salud).
  2. The Referral: You must ask them to refer you to the infectious diseases unit (enfermedades infecciosas) or preventative medicine unit at your local regional hospital for PrEP or full STI screening.
  3. The Wait: Wait times for these hospital appointments outside the major cities can be severe.

General practitioners in smaller regional clinics may have very little experience with PrEP or gay sexual health protocols. You may have to be highly persistent and explicitly demand a referral to the hospital's infectious disease unit.

🚨 Emergency PEP

Regardless of what region you are in, the protocol for emergency PEP is the same: go to the Urgencias (Emergency Room) of the largest public hospital in your area.

Do not go to a small local Centro de Salud for PEP. They do not stock the antiretroviral medications. Go directly to the main regional hospital.
