Mental health and sexual health are deeply connected. Spain has a public mental health system, but it is notoriously underfunded, with long waitlists and very little specialized LGBTQ+ competence.

🛋️ Therapy Options
The Public Route
You can request psychological support via your primary care doctor (médico de cabecera), but the reality is grim.
Waitlists for public psychologists in Spain can stretch from 3 to 6 months. When you do get an appointment, sessions are often spaced 4 to 6 weeks apart, which is ineffective for acute issues. Furthermore, you cannot guarantee the therapist will understand gay male sexual dynamics or chemsex.
The NGO Route
Because the public system is lacking, the LGBTQ+ NGO sector steps in.
- Apoyo Positivo (Madrid/Malaga): Offers specialized psychological counseling, particularly around HIV diagnosis, sexual health anxiety, and chemsex recovery.
- Stop Sida (Barcelona): Provides excellent peer support and psychological services.
- COGAM (Madrid) & Casal Lambda (Barcelona): Offer generalized LGBTQ+ counseling and support groups.
Private Therapy
If you can afford it (€50–€90 per session), private therapy is immediate. Spain has a massive private psychology sector. Many therapists specialize in LGBTQ+ issues (psicología afirmativa LGTBIQ+) and offer online sessions in both Spanish and English.
