For the clinical reality of living with HIV, treatment timelines, and what U=U means biologically, see HIV in 2026: The Facts Without the Fear first. This page covers the immediate local pathway in Bulgaria.

The Acute Diagnosis Pathway

A reactive rapid test (e.g., at Checkpoint Sofia) must be confirmed with a laboratory blood test. Once confirmed, your care is transferred to one of the specialized HIV treatment centers (often located within infectious disease hospitals, such as SBALIPB in Sofia or regional equivalents in Plovdiv and Varna).

Treatment and Costs

  • Access to ART: Antiretroviral Therapy (ART) is provided free of charge by the Ministry of Health to all Bulgarian citizens and legal residents, regardless of health insurance status. The specialized centers dispense the medication directly.
  • U=U (Undetectable = Untransmittable): The medical specialists at the HIV treatment centers understand and support U=U. Once your viral load is undetectable, you cannot pass the virus on.

Bulgarian law historically has provisions penalizing the transmission of infectious diseases, but direct criminalization for non-disclosure when undetectable (U=U) is not the standard application of the law. However, stigma remains high, so navigating disclosure is often more of a social and personal safety issue than a strictly legal one.

Peer Support

An HIV diagnosis in Bulgaria can feel isolating due to societal stigma. Single Step Foundation and Checkpoint Sofia are vital here. They offer dedicated psychological support, peer groups, and social workers who can help you navigate both the medical system and the emotional impact of the diagnosis.
