The Short Version

PrEP in Sweden is not free outright, but it is covered by the national medicine benefit scheme (Läkemedelsförmånen). Your total out-of-pocket cost for all prescription medications combined is capped at roughly 2,900 SEK per year — once you reach that cap, PrEP and all other prescriptions are free for the rest of the year. In practice, many people reach the cap through other medications, making PrEP effectively free.

The best entry point is Venhälsan at Södersjukhuset. There is often a waiting list for first appointments.

The Cost Structure

Sweden's medicine benefit scheme works on an annual accumulation basis:

  • You pay the full prescription cost until cumulative spending reaches approximately 1,150 SEK (reduced cost starts here)
  • As you spend more, each prescription costs less
  • Once cumulative spending hits approximately 2,900 SEK in a 12-month period, all further prescriptions are free for the rest of that period

Generic tenofovir/emtricitabine (the main PrEP medication) costs roughly 250–500 SEK per month before the cap — manageable for most people, and often free if you have other regular prescriptions.

Doctor consultations for PrEP monitoring are free under the Smittskyddslagen, because they are classified as infectious disease prevention care.

Where to Access PrEP

Venhälsan (Stockholm) — primary option Södersjukhuset, Stockholm Phone: 08-616 37 20 Website: 1177.se

Venhälsan is the specialist centre for gay and bisexual men's health in Stockholm. Self-referral is possible — you do not need a GP referral (remiss) to contact them directly. Book via phone or 1177.se.

Waiting times for the first appointment have varied — sometimes several weeks. Once in the programme, follow-up appointments are straightforward.

Infektionsmottagning (Infectious Disease Outpatient) — other cities In cities without a dedicated clinic like Venhälsan, the Infektionsmottagning at the main regional hospital handles PrEP. Ask for "PrEP-behandling" when calling to book.

Sesam mottagning / STI clinics Some regional sexual health clinics also prescribe PrEP. Varies by region — check via 1177.se for your area.

The Process

Step 1: Contact Venhälsan by phone (or equivalent in your region) and request a PrEP assessment appointment

Step 2: Pre-PrEP blood tests:

  • HIV (negative required)
  • Creatinine / kidney function
  • Hepatitis B status
  • STI screen (including three-site swabs)

Step 3: Consultation with a doctor — Venhälsan will assess your risk factors and prescribe PrEP

Step 4: Collect from a Swedish pharmacy (apotek) with your prescription — the medicine benefit scheme applies automatically

Step 5: Follow-up every 3 months for HIV test, creatinine, and STI screen. Doctor consultations are free; prescription co-pay applies until you reach the annual cap.

On-Demand PrEP

The 2+1+1 on-demand protocol is recognised in Sweden and Venhälsan staff are familiar with it. If you prefer on-demand dosing, tell your prescriber — it reduces the number of pills and may reduce costs further.

If You Do Not Have a Personnummer

Without a Swedish Personnummer, accessing the Läkemedelsförmånen medicine benefit scheme is more difficult. Options:

  • RFSL can sometimes facilitate access to PrEP advice for visitors and people without Personnummer — check rfsl.se
  • EU visitors can access some care under EHIC, but ongoing PrEP management requires residence
  • For short-term visitors, private purchase of generic tenofovir/emtricitabine through an EU pharmacy (e.g., from your home country) may be more practical
