The Context
Chemsex — the use of specific drugs (crystal methamphetamine, mephedrone, GHB/GBL, and sometimes ketamine) during sex, predominantly among gay and bisexual men — exists in Romania as in every European country. The context in Romania is one of high social stigma around both homosexuality and drug use, limited LGBTQ+-specific harm reduction resources, and a criminal justice approach to drugs rather than a harm reduction approach.
Personal drug use is illegal in Romania and possession can result in prosecution — this is different from Portugal or some other EU countries. This creates real barriers to seeking help. However, healthcare providers and ARAS are bound by confidentiality; your conversations with them cannot be used against you criminally.

ARAS staff are the most appropriate first point of contact for chemsex concerns. They have training in harm reduction and work with gay and bisexual men in non-judgmental settings. They can:
- Discuss your situation confidentially
- Provide harm reduction information
- Help you access STI testing after sessions
- Refer to addiction specialists if you want to reduce or stop use
- Advise on PrEP adherence during chemsex
ARAS operates under professional confidentiality — what you discuss there does not go to police.
Harm Reduction Essentials
HIV and STI Risk
Chemsex significantly elevates HIV and STI transmission risk — through extended sessions, multiple partners, and reduced attention to condom use. Romania already has one of the highest HIV rates in the EU. Regular testing after chemsex episodes is essential:
- Test for HIV, syphilis, gonorrhoea, and chlamydia after every significant episode
- Request three-site swabs: throat (exsudat faringian), rectum (exsudat anal), urethra (exsudat uretral)
- Keep PrEP going — missing doses during chemsex sessions reduces protection
GHB/GBL — The Overdose Risk
GHB and GBL have a narrow gap between an effective dose and unconsciousness. Mixing with alcohol multiplies this risk and can cause respiratory failure.
Rules that can prevent death:
- Never mix GHB/GBL with alcohol — this combination kills
- Space doses at least 2–3 hours apart
- If someone becomes unresponsive, call 112 immediately — say they have lost consciousness; you do not need to specify what they took to get an ambulance
- Tell paramedics what was taken — they cannot treat effectively without this information
Crystal Meth and Mephedrone
Both are stimulants that extend arousal and wakefulness over many hours. Risks include cardiovascular stress, severe post-session depression and anxiety ("comedown"), and rapid psychological dependency with regular use. If you are using frequently and feel it is becoming difficult to control, ARAS can refer you to an addiction specialist.
Injecting (Slamming)
If drugs are being injected, never share needles, syringes, or any injecting equipment. ARAS operates a needle exchange. Call 021 252 43 70.

PrEP During Chemsex
If you are on PrEP and engage in chemsex:
- Daily PrEP maintains protection even during extended sessions if doses are taken consistently
- If you miss doses during a session, assess the risk exposure and consider PEP if significant
- If you use on-demand PrEP (2+1+1), the pre-sex doses must actually be taken for protection — harder to manage during spontaneous or extended sessions
- ARAS can advise on which PrEP approach fits your pattern of use
