Lithuania's sexual health resources for gay and bisexual men are concentrated in Vilnius. Outside the capital, you're primarily relying on general hospital services, private labs, and LGL's remote support. Here's what's available.
Kaunas
Kaunas is Lithuania's second largest city and has its own university hospital with infectious disease capacity.

PrEP: Kauno Klinikos has an infectious disease outpatient department (infektologijos poliklinika) that prescribes PrEP. Kaunas-based gay and bisexual men do not need to travel to Vilnius for PrEP — book an appointment with the infektologas at Kauno Klinikos.
PEP: Go to the Priėmimo skyrius (emergency reception) at Kauno Klinikos. Say: "ŽIV ekspozicija — reikia PEP."
STI testing: Kauno Klinikos outpatient departments and private labs (Antėja, Medicina Practica network) are your options. No community testing centre equivalent to Demetra operates in Kaunas.
Community: LGL (lgl.lt) can provide remote support and may know of community contacts in Kaunas.
Klaipėda
Klaipėda (Lithuania's main port city) is Lithuania's third largest city.

For PEP, go to the Priėmimo skyrius and request the infektologas dežurinis (on-call infectious disease specialist).
For PrEP: the infectious disease outpatient service at Klaipėdos ligoninė can arrange prescriptions — call ahead to confirm current availability. Some patients travel to Vilnius for initial PrEP consultation and then manage prescriptions locally.
Private labs for STI testing are available in Klaipėda city centre.
Šiauliai and Panevėžys
Lithuania's fourth and fifth cities have regional hospitals (Šiaulių ligoninė, Panevėžio ligoninė) with emergency departments where PEP can be initiated. For routine sexual health care and PrEP, travelling to Vilnius or Kaunas will provide better access.
Practical Advice for Non-Vilnius Residents
LGL is your remote support. Even if you're in Kaunas or Klaipėda, LGL (lgl.lt) can provide guidance, referrals, and peer connection by phone and online. They're aware of the access gap outside Vilnius and do their best to navigate it.
Know your nearest ligoninė for PEP. Have the Priėmimo skyrius number saved before you need it. Every significant Lithuanian city has a hospital that can handle PEP emergencies.
Private labs for routine testing. Antėja and Medicina Practica both operate in major Lithuanian cities. For HIV and syphilis monitoring between Demetra visits, private labs are fast and discreet.
