Mental health crisis support in Bulgaria is less developed than in Western Europe. If you are in a mental health crisis, the hospital emergency system is the immediate route — ask for a psychiatrist on call (психиатър на повикване).
Single Step Foundation (singlestep.bg) can provide emotional support and referrals to mental health professionals during business hours.
LGBTQ+-Specific Support
Single Step Foundation (Фондация „Единична стъпка")
Bulgaria's leading LGBTQ+-aware HIV and health NGO. While primarily a sexual health organisation, Single Step has counsellors and social workers who can provide support and referrals for gay men facing mental health challenges, including post-diagnosis support and crisis navigation.

Bulgarian Helsinki Committee (Български хелзинкски комитет)
Monitors and advocates on human rights including LGBTQ+ rights in Bulgaria. Can provide legal support if you've faced discrimination. Not a mental health service, but a key resource if experiences of discrimination are at the root of distress.

Mental Health Through the Health System
Psychologist / Psychiatrist Access
Bulgaria has both public (НЗОК-contracted) and private psychologists and psychiatrists. NHIF-covered mental health sessions are available but the public system is underfunded and waiting times can be long. Private psychologists (частни психолози) are more accessible, typically charging 40–80 BGN per session (€20–40) — significantly cheaper than Western Europe.

Finding an LGBTQ+-affirming therapist: The Bulgarian psychotherapy landscape has limited formal LGBTQ+-specific training. Useful screening questions (in English if the therapist speaks it, or Bulgarian):
- "Имате ли опит в работата с ЛГБТК+ клиенти?" — Do you have experience working with LGBTQ+ clients?
- "Имате ли религиозни убеждения, които влияят на вашата практика?" — Do you have religious beliefs that influence your practice?
The Bulgarian Context: Stigma Is Real
Bulgaria has one of the more challenging LGBTQ+ environments in the EU. Same-sex partnerships are not legally recognised. "Gender ideology" has been a political flashpoint. Sofia Pride has faced organised opposition since its inception in 2008. Outside Sofia, the environment is significantly more conservative.
This has direct mental health consequences. Research consistently shows higher rates of depression, anxiety, and suicidal ideation among LGBTQ+ people in higher-stigma environments. The minority stress experienced by gay and bisexual men in Bulgaria is a legitimate clinical consideration, not a personal weakness.

Specific Situations
HIV Diagnosis
SBALIPB (the Infectious Disease Hospital) has social workers who can facilitate initial support. Single Step Foundation is the most important community resource — they work with newly diagnosed gay men regularly and can provide peer support and practical navigation.
