The Insurance System

Mutuelle / Ziekenfonds (Health Insurance Fund)

Belgium's health system is built around Mutuelles (French) or Ziekenfondsen (Dutch) — mutual health insurance funds that every Belgian resident should be enrolled in. These are not private insurers but quasi-public non-profit funds. Main providers:

  • CM (Christelijke Mutualiteit) — Flemish, largest overall
  • Solidaris — French-speaking; historically socialist-movement affiliated
  • Partenamut — French-speaking
  • Liberale Mutualiteit / Mutualité Libérale — liberal-affiliated
  • Neutraal Ziekenfonds / Mutualité Neutre — non-pillar-aligned

For new residents, registering with a Mutuelle is a day-one priority. You need your eID card and Belgian National Register number. Without Mutuelle membership, healthcare costs are dramatically higher.

Remboursement / Terugbetaling (Reimbursement)

Belgium's system is not free at point of use — you typically pay the full amount upfront at a doctor or pharmacy, and the Mutuelle reimburses the covered portion. The ticket modérateur is the residual co-payment you keep.

Ticket Modérateur / Remgeld (Co-payment)

The portion of the healthcare bill you personally pay after reimbursement. For standard GP visits: typically €1–5. For specialist visits and A&E: €10–30. For PrEP via the Convention: €8–12/month.

HRC (HIV Reference Center)

The specialist HIV and sexual health units attached to Belgian university hospitals. Only HRCs can initiate PrEP Convention reimbursement. They also manage HIV treatment, complex STI cases, and co-ordinate vaccinations for at-risk groups. There are approximately 20 HRCs across Belgium — full list at aids.be.

Convention (PrEP Convention)

The formal enrolment contract signed between a patient and an HRC that enables reimbursed PrEP access. Requires signing at an HRC and committing to three-monthly monitoring appointments. Without a Convention, PrEP costs €200+/month at full pharmacy price.

eID (Elektronische Identiteitskaart / Carte d'identité électronique)

Belgium's electronic identity card. Used to access health services, pharmacy reimbursements, and administrative processes. Keep it on you at all medical appointments.
